Qualified Intellectual Disability Professional
A Qualified Intellectual Disability Professional is directly accountable for the assessment, development, implementation, and monitoring of a highly individualized Individual Support Plan (ISP) which promotes dignity, respect, choice, and the exercising of personal rights for each person who is on their assigned caseload. The Individual Support Plan (ISP) for each person must be developed by the person's appropriately constituted Interdisciplinary Team (IDT) through an integrated, collaborative and comprehensive program planning process that reflects the person's preferences, strengths, needs and personal vision. The QIDP-III coordinates the development of programs and training to implement the individual plan for services and supports. Must independently implement Federal/State Standards (ICF/IID), the requirements of the Settlement Agreement with the Department of Justice (DOJ) and the Center's Policies and Procedures. The QIDP-III provides complex professional and administrative support, and technical assistance work that is designed to address specific needs, desires, and personal goals of those who live in the assigned home(s). The QIDP-III works with and interprets needs based on input from a variety of service providers, professionals and clinicians. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Work schedule requires regular flexibility, to ensure effective oversight and monitoring of active treatment programs, services and supports. If needed, may supervise the work of others.Essential Job Functions: Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. 1. Integrates and coordinates the individual support plan of each individual. 2. Ensures participation of appropriate disciplines in planning meetings. 3. Ensures completion of the Comprehensive Functional Assessment. 4. Ensures that each individual receives necessary services and interventions to promote optimum quality of life. 5. All services and programs are provided as specified in the ISP. 6. Monitors the active treatment program. 7. Documents a comprehensive program review on a monthly, and annual basis. 8. Revises the ISP as needed. 9. Coordinates incident management for each individual. 10. Advocates for the individual. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.Knowledge Skills Abilities: Demonstrates the following: 1.Knowledge of the Individual Support Plan (ISP) process. 2.Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities, including program design and collecting and analyzing data. 3.Ability to coordinate programmatic and clinical services and supports.4.Ability to prepare and maintain records.5.Ability to communicate verbally and in writing in a clear and constructive manner. 6.Ability to assess and solve problems. 7.Ability to work independently and organize/prioritize work.8.Ability to work harmoniously and professionally with staff at all levels 9.Ability to physically perform direct care work as needed, including emergency procedures.10.Skills in facilitating Interdisciplinary Team (IDT) meetings to ensure participation by all members and achieve desired outcomes.11.Skill in using a personal computer to create documents and reports.Registration or Licensure Requirements: Initial Selection Criteria: At least Bachelor's degree from an accredited college or university with a major in psychology, sociology, special education, rehabilitation counseling, social work or a related human services field.At least one (1) year experience working directly with persons with intellectual di
